Application of CT three-dimensional reconstruction combined with 3D printing in laparoscopic nephron-sparing surgery

摘要: 目的:探讨CT三维重建联合3D打印技术在腹腔镜下肾部分切除手术中的应用价值。方法:我院在2016年10月~2018年12月收治的58例T1期肾肿瘤患者,其中27例术前行CT三维重建并应用3D打印技术打印模型,31例不应用此项技术,比较两种方法的手术时间、热缺血时间、术中失血量、住院时间、肌酐变化等指标。结果:58例手术均成功完成,无中转开放。3D打印组手术时间、热缺血时间较常规组短,差异有统计学意义,两组在术中失血量、切缘阳性率,术前术后肌酐变化等方面比较差异无统计学意义(P>0.05)。结论:3D打印技术可以提高肾部分切除术手术效率,降低手术风险,值得临床推广。Abstract: Objective: To investigate the value of CT three-dimensional reconstruction combined with 3 D printing in laparoscopic partial nephrectomy. Method: From October 2016 to December 2018, 58 patients with stage T1renal tumors were treated in our hospital. Twenty-seven of them underwent CT three-dimensional reconstruction and 3 D printing before operation, while other 31 patients did not use this technique. The operation time, warm ischemia time, intraoperative blood loss, hospitalization time, creatinine change and positive rate of incision margin were compared between the two methods. Result: All 58 cases were successfully operated without conversion to open surgery. The operation time and warm ischemia time in the 3 D printing group were shorter than those in the conventional group. However, there was no significant difference in intraoperative blood loss, positive rate of incision margin or creatinine change before and after operation between the two groups(P>0.05).Conclusion: Three-dimensional printing technology can improve the efficiency of partial nephrectomy and reduce the risk of operation, so it is worthy of clinical promotion.
