单孔腹腔镜技术在泌尿外科的应用进展及展望

    刘存东,主任医师,副教授,博士生导师,南方医科大学第三附属医院泌尿外科主任。现任中华医学会泌尿外科分会微创学组委员、广东省医学会泌尿外科分会副主任委员、广东省泌尿生殖协会前列腺疾病学分会主任委员、中华医学会男科分会前列腺疾病学组委员、海峡两岸医药卫生交流协会泌尿外科专业委员会单孔学组组长、中国医师协会男科医师分会生殖肿瘤委员会委员。《临床泌尿外科杂志》《现代泌尿生殖肿瘤杂志》编委,南方医科大学学报特约编委。擅长单孔腹腔镜技术完成各类根治性手术:肾癌、肾盂癌、输尿管癌、膀胱癌、前列腺癌的根治,包括保留肾单位的肾肿瘤切除等;单孔腹腔镜完成各类矫形或重建手术:肾盂输尿管连接部狭窄、巨输尿管畸形、重复输尿管畸形、输尿管狭窄段切除再吻合术、输尿管膀胱再植术、膀胱阴道瘘修补术等;单孔腹腔镜完成的其他手术:单孔经膀胱前列腺剜除术、膀胱憩室切除术、肾盂输尿管切开取石术等。主持国家自然科学基金、省自然科学基金及省科技计划项目6项。以第一作者或通讯作者发表文章33篇;SCI收录论文21篇,其中一区论文6篇,二区论文6篇。主编及参编著作4部。被评为广东省杰出青年医学人才,并多次获得南方医科大学标兵个人、南方医科大学四热爱先进个人等荣誉称号

Application progress and future direction of laparo-endoscopic single-site surgery in urology

  • 单孔腹腔镜技术(laparo-endoscopic single-site surgery,LESS)在继承传统腹腔镜技术特点的基础上具有更好的术后美容效果和患者术后恢复更快的独特优势。然而,单孔腹腔镜手术存在暴露不佳、器械碰撞干扰、缝合难度大等难点。自2007年以来,国内外泌尿外科医师不断拓展LESS在泌尿外科领域的应用,通过选择合理的患者体位、选择合适的手术切口、联合自然腔道以及研发新型手术器械等途径来降低单孔腹腔镜手术的操作难度,从而推动单孔腹腔镜手术的广泛开展。机器人辅助单孔腹腔镜手术的开展更加彰显单孔腹腔镜手术的独特优势,将显著推动LESS的快速发展和广泛应用。本文将综述LESS在泌尿外科的发展历程,并对LESS在泌尿外科的应用进展及未来发展方向作一述评。
