Clinical comparative study on different kinds of tubeless PCNL in the treatment of patients with renal calculi

摘要: 目的:探讨部分和完全无管化经皮肾镜取石术(PCNL)治疗肾结石临床疗效差异,为手术适应证选择提供更多临床证据。方法:选取我院2016年1月~2018年7月收治符合手术指征肾结石患者共130例,采用部分无管化PCNL治疗的65例患者为部分组,采用完全无管化PCNL治疗的65例患者为完全组;比较两组清石效果、手术相关指标水平、手术前后生活质量评分差值及术后并发症发生率。结果:两组清石效果比较差异无统计学意义(P>0.05);完全组手术用时显著少于部分组(P<0.05);两组手术前后Hb差值、肾周积液消失时间、尿外渗消失时间、住院时间及总治疗费用比较差异无统计学意义(P>0.05);完全组手术前后生活质量评分差值显著高于部分组(P<0.05);完全组术后并发症发生率显著低于部分组(P<0.05)。结论:完全无管化PCNL治疗肾结石能够显著降低操作难度,改善生存质量,并有助于预防术后并发症发生,价值优于部分无管化PCNL。Abstract: Objective: To investigate the difference of clinical effects between partial and whole tubeless PCNL in the treatment of patients with renal calculi. Method: From January 2016 to July 2018, 130 patients with renal calculi were chosen in our hospital and divided into partial group(65 patients) undergoing partial tubeless PCNL and whole group(65 patients) undergoing whole tubeless PCNL. The stone removal rate, the levels of operation-related clinical indicators, the difference of quality of life between before and after operation and postoperative complication were compared between two groups. Result: There was no significant difference in the stone removal rate between two groups(P>0.05). The operation time of whole group was significantly shorter than that of partial group(P<0.05). There was no significant difference in the Hb decrease difference value before and after operation, the disappeared time of perirenal effusion, extravasation, total hospitalization time and total treatment costs between two groups(P<0.05). The difference of quality of life between before and after operation of whole group was significantly higher than that of partial group(P<0.05). The postoperative complication of whole group was significantly lower than that of partial group(P<0.05).Conclusion: Compared with partial tubeless PCNL, whole tubeless PCNL in the treatment of patients with renal calculi can efficiently reduce the difficulty of operation, improve the quality of life and be helpful to prevent postoperative complications.
