One case of rectal cancer metastasizing to the prostate 7 years after radical operation

摘要: 探讨直肠癌术后转移至前列腺的临床表现、诊断及治疗,以提高医师对此认识。南京医科大学附属逸夫医院2020年5月5日收治1例直肠癌前列腺转移男性病例,对相关文献进行复习。结合直肠腺癌病史、前列腺及病变组织切片及免疫组织化学染色病理确诊为直肠腺癌前列腺转移。对于有结直肠癌病史特别是结直肠癌临床分期较高、淋巴结转移或血管侵犯等风险因素的患者,应密切关注并定期进行检查。若患者出现排尿困难、尿频、尿急或血尿等与前列腺相关的症状,应警惕结直肠癌前列腺转移可能,及时进行评估和进一步检查,完善病理或影像学检查明确诊断,避免误诊误治并根据患者的病情选择个体化的治疗方案,结合化疗、免疫治疗、靶向治疗、局部放疗或手术。Abstract: The clinical manifestations, diagnosis, and treatment of postoperative metastasis of rectal cancer to the prostate were explored in order to enhance surgeons' understanding of this disease. A male case of rectal cancer with prostate metastasis was admitted to Sir Run Run Hospital of Nanjing Medical University on May 5th, 2020, then we reviewed relevant literature. Combined with the history of rectal adenocarcinoma, prostate and lesion tissue sections, and immunohistochemical staining pathology, he was confirmed as rectal adenocarcinoma with prostate metastasis. For patients with a history of colorectal cancer, especially those with risk factors such as high clinical stage of colorectal cancer, lymph node metastasis, or vascular invasion, close attention should be paid to and regular inspections should be performed. If the patient has symptoms related to the prostate, such as dysuria, frequent urination, urgency or hematuria, he should be alert to the possibility of prostate metastasis from colorectal cancer and conducted with timely assessment and further examination. Also, the pathological or imaging examination should be completed to confirm the diagnosis and avoid misdiagnosis and mistreatment. An individualized treatment plan was chosen according to the patient's condition, including chemotherapy, immunotherapy, targeted therapy, local radiotherapy or surgery.
