Application of artificial hydronephrosis with minimally invasive percutaneous nephrolithotripsy in management of calyceal diverticular calculi without hydronephrosis

摘要: 目的:探讨微创经皮肾镜碎石术(MPCNL)治疗肾盏憩室结石的可行性与有效性。方法:回顾性分析2011年3月~2013年3月应用MPCNL治疗肾盏憩室结石患者58例的临床资料:采用C臂X线机辅助穿刺建立经皮肾通道,应用气压弹道碎石并取出,同时术中视具体情况采用电凝憩室囊壁或扩张憩室盏颈。结果:手术时间(107±43)min,一次性取尽无残石49例,占84.5%。平均住院7天,出院时症状缓解53例,症状减轻5例。结论:MPCNL治疗合并临床症状的肾盏憩室结石疗效确切,创伤小,恢复快,可反复操作,可作为临床选择的治疗方式之一。Abstract: Objective: To investigate the feasibility and efficacy of MPCNL for the treatment of calyceal diverticular calculi.Method: Clinical data of 58 cases from March 2011 to March 2013 who underwent minimally invasive percutaneous nephrolithotripsy (MPCNL) were summarized and analyzed retrospectively. Under the guide of C-arm X-ray apparatus, we performed pneumatic lithotripsy and the diverticulum was managed by electrically coagulating the diverticular wall or dilating the diverticular neck. Result: Mean operative time of all patients was (107±43) mins. Stone-free rate was 84.5% (49/58). Mean hospital stay was 7 days. The symptoms of fifty-three cases vanished and five cases decreased.Conclusion: According to our study, MPCNL is effective in treating calyceal diverticular calculi. It could be offered as an alternative therapy for symptomatic calyceal diverticular calculi because of its high stone-free rate, less invasion and satisfied recovery.
