摘要: 总结复式脉冲ESWL治疗上尿路结石的临床资料。方法:回顾性分析6 939例行复式脉冲ESWL术患者的临床资料,肾结石5 381例,输尿管结石1 558例,其中单纯ESWL3 123例,输尿管镜术或经皮肾镜取石术后3 816例,结石大小4 mm×4 mm~26 mm×27 mm。工作电压 3.0~8.5 kV,肾结石平均冲击次数2 177 次,输尿管结石平均冲击次数2 486次。结果:肾结石治愈率59.3%,输尿管结石治愈率68.49%,术后5857例(84.4%)出现血尿,1166例(16.8%)肾绞痛,135例(1.95%)高热,7例(0.1%)肾周围血肿,64例(0.92%)输尿管石街,其中9例输尿管镜取石,31例(0.45%)中止手术未完成治疗。结论:复式脉冲ESWL术创伤小、成功率高、严重并发症少,仍是处理上尿路结石主要的治疗手段之一。Abstract: Objective: To summarize the clinical experience of upper urinary stone treated with duplex pulse ESWL.Methods: Retrospectively analyzed 6 939 cases with upper urinary stone treated with duplex pulse ESWL in our center.5 381 cases was renal calculi,and 1 558 cases was ureteral calculi.3 816 cases had residual stone,which were treated after ureteroscopy or percutaneous nephrolithotomy,and the other 3 123 cases were not deal with the stone by any operation before ESWL.The stone size were from 4 mm×4 mm to 26 mm×27 mm.The working voltage was 3.0 to 8.5 kV,The average number of impulse was 2 177 in Renal calculi,and 2 486 in ureteral calculi.Results: The cure rate of ESWL was 59.3% in renal calculi,and 68.49% in ureteral calculi.After ESWL,5 857 cases (84.4%)were hematuria.1 166 cases (16.8%) were Renal colic,135 cases(1.95%) were hot fever.7 cases (0.1%)were renal subcapsular hematoma.64 cases (0.92%)were Ureteral stone steps and 9 cases were treated by ureteroscopy.31 cases (0.45%)did not complete treatment.Conclusions: Duplex pulse ESWL is especially accurate,and the high success rate in upper urinary stone treatment.The tissue trauma is moderate and no serious side-effect is observed.It is an indispensable minimally invasive treatment for ureteral calculi up to date.
